The secret medal is for getting all the coins in one run. Start by collecting the coin under the crusher and quickly walking back to the starting area, collecting all of the coins in said area, then dropping down the tunnel to the left. After that, just go backwards while collecting every coin.

My final time was 31:16 with 86 deaths. Boss was really hard but I beat it. Fun and classic game
